Reason

Despite reinforcement of the instructions for use for attachment of Q-link 1 strap lock to MultiRall from the ground level with the Extension Arm accessory, serious incidents/adverse events have continued to occur with drop of patient and lift motor. CARSAT a government organization requests changeout to Q-link 2 and usability studies during design development indicate Q-link 2 design was less prone to incorrect attachment due to use error compared to the Q-link 1.
