Issue:
On certain aerial devices, the platform leveling sprocket may not have been properly welded and could fail. If this happens, the platform may not stay level and could tip suddenly.

Safety Risk:
If the platform tips suddenly, a person could slip from the platform and fall, creating the risk of an injury.

Corrective Actions:
Altec Industries will notify owners by mail and provide them with instructions for how to inspect the leveling sprocket. If the sprocket assembly is cracked, it will need to be replaced. If it is not damaged, a secondary retention kit will be installed to limit rotation of the platform if the sprocket weld fails.
